Requesting Prescriptions

Please do not run out of your regular prescriptions. Do think ahead and order the prescription in plenty of time especially at holiday times when the surgery may be closed.  Make your request  2 working days before you collect your prescription. Please bring in or post the Repeat Prescription "tear-off" slip to Reception - marking those items that you need.

We do not accept requests for prescriptions over the telephone as mistakes can be made. Requests can be made in writing but everyone is requested to use the Repeat Prescription "tear-off" slip to reduce the chance of an error.

Some local chemists will collect your prescription from the surgery so that you can collect your prescription items directly from the chemist. If you would like to use this service please ask at Reception or at your local chemist. If you cannot collect the prescription yourself, you will have to give written permission for someone else to collect the prescription for you. This is to prevent duplicate prescriptions and fraud occurring which unfortunately is not uncommon.
