How to Register
In order to register with this Practice, you have to be a resident within a defined area. A map of the practice area can be found by clicking here.
To confirm your address is in the catchment area, you will need to attend the Practice and provide some formal proof of address. This can be either a Utility bill (for example, a gas, electricity or telephone bill), or a bank/credit card statement, mortgage or rent statement or driving license .
A member of the practice staff will explain the practice policy and procedures to you and ask you to read and sign the Agreement to Practice and Registration Policy, and to complete a registration form. Once this is completed you will be asked to make an appointment for a health check with the nurse before we can finalise your registration.
We will only register patients at the following times:
| Tuesday 1.30 to 4.00pm | Thursday 8.00am to 9.30am |
| Wednesday 8.00 to 9.30pm | Friday 1.30pm to 4.00 pm |
