How to Register

In order to register with this Practice, you have to be a resident within a defined area. A map of the practice area can be found by clicking here.

To confirm your address is in the catchment area, you will need to attend the Practice and provide two formal proofs of address. This can be either a Utility bill (for example, a gas, electricity or telephone bill), or a bank/credit card statement, mortgage or rent statement , council tax bill or driving licence. Please also bring your old medical card if you have it or details of your last GP.

A member of the practice staff will explain the practice policy and procedures to you and ask you to read and sign the Agreement to Practice and Registration Policy, and to complete a registration form. Once this is completed you will be asked to make an appointment for a health check with the nurse before we can finalise your registration.

We register patients only at the following times:

 

Mon

2pm  - 6pm  

 Tues

8am  - 12pm

 Weds

8am  - 12pm

 Thurs

 8am - 12pm

 Fri

8am  - 12pm
